Employee Health —

The following forms are required by Employee Health Services and need to be returned by May 15th if at all possible. You must fulfill all immunization requirements prior to orientation. Your pay advance will be held until you have been cleared by Employee Health. If you have any questions, please send an e-mail to nsarver@wfubmc.edu.

Immunizations:
It is likely that you are in compliance with immunizations; however, deficiencies MUST be taken care of prior to your arrival. If not, then you will be directed to report to one of the several emergency health care clinics available in the area (i.e. PrimeCare); or you may make an appointment with our Family Practice Clinic. The cost of medical care is the responsibility of the student.
